The initiative, which launched across all ten boroughs including Oldham, aims to provide residents with better connected care that works seamlessly across different providers.

The partnership connects GPs, hospitals, council services, police, fire services, charities, and private companies to create a unified system that supports people throughout their lives. For Oldham residents, this means healthcare professionals from different organisations will now share information and coordinate care more effectively, reducing the frustration of having to repeat your story to multiple providers.
